Create block/qcow2-bitmap.c
Add data structures and constraints accordingly to docs/specs/qcow2.txt

The spec accepts 0. Should we change the spec or at least add a note
that qemu requires a minimum of 512 bytes?

Misunderstanding already avoided with Qcow2BitmapDirEntry. :-)
