[v6,3/8] block: implement reference count for BlockDriverState

Introduce bdrv_ref/bdrv_unref to manage the lifecycle of
BlockDriverState. They are unused for now but will used to replace
bdrv_delete() later.

Just a heads-up: This won't apply cleanly to master because of a conflict,
at the above line, with
'88266f5 block: stop relying on io_flush() in bdrv_drain_all()'.

Thanks Jeff, rebased locally, the merging is straightforward: just add
this line in the new context. Since it's only a trivial conflict, let's
wait for some more comments before incrementing the revision.
