This is a pull request for the update of Xenial to upstream stable release 4.4.132.

Skipped patches (already applied):
  * KVM: s390: Enable all facility bits that are known good for passthrough

All patches applied cleanly except for a few that required trivial context
adjustments or touched files that are renamed/moved in Xenial.

Compile-tested all supported architectures.
