[v8,03/14] hw/arm/smmu-common: VMSAv8-64 page table walk

This patch implements the page table walk for VMSAv8-64.

- as the translate function was not straightforward to me,
  I tried to stick more closely to the VMSA spec.
- remove support of nested stage (kernel driver does not
  support it anyway)
- use error_report and trace events
- add aa64[] field in SMMUTransCfg
