Implement -Wimplicit-fallthrough: core

Since my last attempt to attack this beast wasn't exactly successful,
I'm offering a better version.  For the general introduction regarding
what and why, kindly see

What has changed:
* I renamed the option to -Wimplicit-fallthrough.  While I don't like this
  name that much, it's what clang uses, so let's follow their naming.
* we now take various /* fall thru */ comments into account.  This has
  significantly reduced the verbosity of the warning.
* several people raised concerns about __builtin_fallthrough.  I changed
  my mind and instead of that builtin I implemented
  __attribute__((fallthrough)), and for C++ also [[fallthrough]] (for <C++17
  [[gnu::fallthrough]]).  This attribute appertains to a null statement,
  which I think is a whole new category of attributes, so I documented that.
  It gets transformed into an internal function in the FEs.
* the warning is enabled by -Wextra.
* the warning doesn't warn for cases like
  ... something; default:; }
  ... something; default: break; }
  in outermost switches
* I added fixit hints, so the warning also says
    insert '__attribute__ ((fallthrough));' to silence this warning
    insert 'break;' to avoid fall-through
* a bunch of bugfixes (e.g. the ICE Martin S. pointed out), more tests.

What still needs to be done:
* solve the behavior with -E / -C / -CC 
* I noticed that we don't diagnose e.g. '__attribute__((noreturn));' in
  the C FE in certain contexts.  Shouldn't be a showstopper though.
* do some experiment as to whether this warning can change the generated
  code in any way

This patch has been tested on powerpc64le-unknown-linux-gnu, aarch64-linux-gnu,
and x86_64-redhat-linux.

I'm afraid this is really bad.
Right now, there are 8 and 8 bit bitfields, then 8-bit char, 3 individual
bits, 5 unused bits and 32-bit int, nicely packed into 64-bit word before a
union with pointer members, and the C++ FE lexes everything first, so there
are possibly millions of tokens in memory.
Can't you just make it unsigned int flags : 11; instead?  Or instead
reshuffle the cpplib.h flags?  E.g. I don't see the C++ FE to use the
NO_EXPAND flag, so moving it to the upper byte of the short and moving the
new flag to its bit?  Perhaps that is even better for now.

Just a couple of minor things:

"Might appear" means that it may or may not appear (we don't know),
but the intent is to say that it's allowed to appear at most once.
Such a constraint is more correctly phrased in terms of either may

   The fallthrough attribute may appear at most once.

or

  The fallthrough attribute must not appear more than once.

Same for "might not be mixed with."

In (very light) testing I noticed that in the following, the switch
in h() is diagnosed but the one in g() is not.

Oops, that's right.  Fixed.

Yeah, that is true.  I'm not sure if the warning can reasonably be expected to
handle all such cases and really see through loops like that, most likely not.
It's really just a heuristics.
Fortunately I didn't see any false positives due to that while bootstrapping.

Can you add it as an xfail'd test?

Bug number?  shadow_tag_warned is meant to diagnose empty declarations 
like that.  (That attributes are ignored when they form the entire 
contents of the parentheses in a function declarator is a known and 
documented peculiarity.)

Actually, this was a problem in my previous version of this patch;
shadow_tag_warned indeed warns on such empty declarations.  I've
fixed that now.  Which means that for

  if (i)
    {
      __attribute__((noreturn));
    }

we'd issue "warning: empty declaration", with the -Wimplicit-fallthrough
patch we'd say
error: only attribute ‘fallthrough’ can be used before ‘;’

Not sure if error is appropriate here, or whether I should downgrade the
error to a warning and ignore the attribute.
With __attribute__((fallthrough)) the snippet above of course compiles
fine as this is a correct usage of the attribute on a null statement.

I'd say the semantics for uses of attributes that currently parse should 
remain unchanged: they should not be reinterpreted (for non-fallthrough 
attributes) to be attributes on a statement rather than on an empty 
declaration.

Thanks, makes sense.  I've done so now.  I'll post the most recent version
of the -Wimplicit-fallthrough patch after it passes testing on a few systems.
