[Ada] Update documentation of debug switches

This patch removes the documentation of debug switches that are not used
anymore by gnat2why, and are thus free for other uses.

In frontend.adb, there was a now obsolete reference to -gnatd.H, which
is also removed.

Tested on x86_64-pc-linux-gnu, committed on trunk
