Cornelia Huck - Sept. 4, 2012, 3:13 p.m.
Add a driver for kvm guests that matches virtual ccw devices provided
by the host as virtio bridge devices.

These virtio-ccw devices use a special set of channel commands in order
to perform virtio functions.

Signed-off-by: Cornelia Huck <cornelia.huck@de.ibm.com>

Changes v1->v2:
- adapt to improved virtio-ccw channel commands
- fix unregistration of online devices
- add a missing spinlock initialization
