[RFC] ext4: Remove old legacy block allocator

This patch removes the old legacy block allocator from ext4.  It shrinks
the compiled size of ext4dev by over 6.5k (bringing it down to 149k) and
removes 1,525 lines of code.  (In comparison, ext3's compiled size is 86k).

Removing the legacy block allocator also exposed a few places where ext4
code was still assuming the old block allocator and wasn't properly
interfaced to mballoc.  In particular, the online resizing code is still
calling ext4_free_blocks_sb(), which is probably not right, and
ext4_find_goal() wasn't properly getting the goal block when mballoc was
being used.

This patch also removed some greatly confusing function names, such as
ext4_old_new_blocks().
