Add bs field to BdrvDirtyBitmap structure. Drop BlockDriverState
parameter from bitmap APIs where possible.

I've thought about doing this before, but couldn't figure out if it was
worth it.

So, let's do it.

makes any functions that take (bs, bitmap) pairs less error-prone.
